What Should a Hospital Estimate in India Include? A Checklist

Written by the HealYourTrip editorial team ยท General information, not medical advice. Editorial policy

When you ask an Indian hospital what your treatment will cost, you usually receive a short PDF with one number, sometimes a range. That number is where most billing disputes begin. Patients travelling from Iraq, Bangladesh and elsewhere have described estimates that left out implants, extra hospital days or items they only discovered on the final bill.

This guide explains what a complete estimate should contain, which costs usually sit outside it, and what to ask before you book a flight. It applies whichever hospital you choose โ€” including hospitals HealYourTrip does not work with.

Why a single total is not enough

An estimate is not a final bill. Hospitals prepare it from your medical reports and the planned procedure, and the final amount can change if your stay is longer, the surgical plan changes or a complication needs extra care. That is normal. A good estimate makes those changes predictable, because it shows:

  • What the price assumes โ€” the procedure, the room type and the number of days
  • What it includes, line by line
  • What it excludes
  • What happens to the price if you stay longer or need something extra

If your estimate only shows a total, ask for the breakdown before deciding.

Line items a complete estimate should show

Item What to check
Procedure or package name Is it exactly the procedure your doctor recommended โ€” for example single vs bilateral knee replacement, or bypass (CABG) vs valve replacement?
Surgeon and anaesthetist fees Included in the package, or billed separately?
Operating theatre charges Usually included in a package โ€” confirm it.
Implants, devices, stents, prosthetics Brand, model and price per unit. This is often the largest variable. Ask whether a different brand changes the price.
Room category and number of days Ward, twin-sharing or private room. How many ICU days and how many ward days? What does each extra day cost?
Medicines and consumables Included up to a limit, or billed at actual cost?
Investigations Which tests before and after surgery are included? Pre-admission tests are often extra.
Physiotherapy and rehabilitation Are inpatient sessions included? Outpatient sessions after discharge?
Blood and blood products Usually billed separately.
Attendant or companion Are meals and a bed for one companion included?
Follow-up consultations How many reviews are included before you are cleared to fly home?
Currency and validity Quoted in INR or USD? How long is the estimate valid?

Costs that usually sit outside the hospital estimate

Budget separately for:

  • Flights for the patient and companion
  • Visa fees โ€” see our India Medical Visa guide
  • Accommodation before admission and after discharge โ€” ask the hospital how long you need to stay nearby for your procedure
  • Local transport, food, a local SIM card, and an interpreter if the hospital does not provide one
  • Medicines to take home
  • Treatment of any other condition found during pre-surgery tests

Questions to ask before you accept an estimate

  • Is this a package price or billed at actuals? If it is a package, what are its limits (days, implant category, medicine cap)?
  • Which specific implant or device is included, and what are the alternatives and their prices?
  • What does each extra day cost in the ICU and in the ward?
  • What is excluded? Please list it in writing.
  • If the plan changes during surgery, who approves the extra cost, and how will I be told?
  • Do I pay a deposit on admission? Is it refundable if surgery does not go ahead?
  • Will I receive an itemised final bill?
  • Is the price the same if I contact the hospital directly?

Comparing estimates from different hospitals

Only compare estimates that assume the same procedure, room category, number of days and implant class. A lower total that assumes a shorter stay or a basic implant is not cheaper โ€” it is a different plan. If in doubt, send all your estimates back to each hospital and ask them to quote on the same basis.
